Choose this appointment if:

• You are a new patient to In Focus Physiotherapy

• A returning patient who has not been in the clinic in the past 3 months, or

• A returning patient with a new injury

Standard Appointment - Senior Physiotherapist ($100 - 30 mins)

This appointment type is for clients currently in treatment with In Focus Physiotherapy.

If you have not had an appointment for more than 3 months, or have a new or additional problem you need reviewed, please book in for an initial consultation

Pensioner and Health Care Card holders will receive $10 off the regular appointment fee.

If you have a chronic and/or persistent condition (a medical condition that has been or will be present for a minimum of 6 months) you may be eligible to access a Chronic Disease Management (CDM) Program. This includes Medicare rebates for up to 5x Allied Health visits (i.e. physiotherapy) per year.

Medicare will provide a $58.00 rebate after full payment of service.

**If you hold a valid pensioner or health care card, your gap will be reduced by $10.

In Focus Physiotherapy accepts WorkCover WA and Motor Vehicle Accident Claims.

To access our services, you will need the following details in order to start your physiotherapy treatment for WorkCover WA:

• Claim acceptance letter from your relevant insuring company from work

• Claim details – including claim number, case manager name and contact details

• GP referral letter

For Motor Vehicle Accident Claims, you will require:

• GP referral letter

• CTP (Compulsory Third Party) letter of acceptance with a claim number

• Your insurance company details including case manager name, contact details (email, address, phone number) and billing details

• Any relevant tests or scans

In Focus Physiotherapy accepts DVA Gold or White card for full payment of services and will bill DVA directly for any care provided to you under DVA arrangements. This will ensure that you do not incur any out pocket expenses.

To access our services, you will require a referral issued by a general practitioner (GP). In certain circumstances, a referral may also be issued by a medical specialist, or a health professional as part of a hospital discharge.

All patients receive an email at the time their appointment is made and a reminder text the day before their appointment.

We ask to please consider others and provide us with 24 hours notice if you need to cancel or reschedule your appointment.

Late cancellations, late reschedules and non-attendance will be charged a full cancellation fee except under exceptional circumstances.
